A HALO Success Story - One Year On!

A HALO Success Story - One Year On!

On Friday 28 June, we celebrated our customers’ Sarah, Rachael, and Gemma living in their Mount Barker property for one year!

We invited along staff, family and friends to the property to celebrate. Everyone was encouraged to bring a plate, and together they enjoyed the festivities, which included speeches, cake and a delicious BBQ dinner. We thank everyone that came along, including the Mentors and Housing and Living Options Coordinators.

We are proud of Sarah, Rachael and Gemma for reaching this fantastic milestone - they are great friends and enjoy each other’s company!

Sarah, Rachael and Gemma are undoubtedly one of our success stories in the Home and Living Options space. All three had personal goals to transition out of home for the very first time. They had previously discussed their goals with several NDIS providers to no avail. Access4u worked with these three customers individually, through an extensive exploration phase. This involved developing a multidisciplinary team and doing several functional capacity assessments, to ensure all three were ready and equipped to move out of home.

Sarah, Rachael and Gemma were previously unknown to one another–met a number of times through meet and greet sessions, day trips and short-term accommodation stays, to get to know one another and to explore whether living together would be suitable. Their families were also heavily involved throughout the process.

After an 8-month process, they moved into their current house -- a house that ultimately suits their needs, wants and goals. They absolutely love their home and have a great dynamic, and we will continue to support them as they progress along their journey - one year on, and beyond!
