This service is suitable for those requiring support and assistance with care outside of the home for children and young people with a disability. The support and assistance available under this model may include regular family support, overnight support, transport, community activities, and school hours care, school holiday support and more.

This care service can assist families and caregivers by providing flexible supports that are designed to improve family relationships whilst the family maintains responsibility for decisions in the young person’s life in partnership with the service provider.
